Making an appointment
Your first appointment with our physical therapy is scheduled for 1 hour. Please arrive 15 minutes early to fill out paperwork. Bring your medical insurance card, your physical therapy prescription and wear comfortable clothing.

If you are a worker’s compensation patient, please call with the following information when making your appointment: your diagnosis, employer name, case number, name of adjustor and phone/fax number of adjustor. We will request authorization for physical therapy.

First time visit to the water clinic
For your first visit at the water clinic please keep in mind that you should arrive about 20 minutes early to rinse off in the shower and put your personal items away before your appointment. Omega runs right on time so plan to be in the therapy section of the pool at the start time of your appointment.

Things you will need to bring to your first water appointment:
·         Towel
·         Water Bottle
·         Bathing Suit
·         Water Shoes (optional)
·         Lock for Locker
·         Change of Clothes
           – Reusable Cloth Water Diaper (for those and children who need them)

Cancelling an appointment
We require 24 hour notice if you can not make your appointment. There will be a $25 charge for any cancellation without proper notice. Please note that this charge is not covered by insurance.

Reminders
We call to remind our patients of future appointments. However, it is the patient’s responsiblity to call to cancel an appointment to avoid a missed appointment fee.
